Keep your student records current by reviewing your personal information on CampusConnection. Some personal information can be updated in CampusConnection whereas other require the submission of appropriate form and documentation. Please see below for specific requirements.

Steps for Requesting a Name Change

Step 1 — Gather documents needed for a name change

  • All name change requests must be accompanied by the corresponding legal documentation showing the change and a signed social security card matching the new name. 

Step 2 — Submit the Legal Name Change Request Form

  • Sign into CampusConnection
  • Select 'NDSCS eForms' tile
  • Select 'Enrollment Services' in the left menu
  • Select 'Legal Name Change Request'

Update Social Security Number

North Dakota State College of Science is required by law to provide the IRS and the student a 1098-T tuition statement. Included on the statement is the student's social security number for tax purposes. Providing your social security number to NDSCS will avoid possible penalties from the IRS.

Also, if you are completing the FAFSA application, a social security number is required on the federal application to receive your federal grants and loans. In order for NDSCS to receive your federal information and begin the awarding process a social security number needs to be on file.

Steps to updating an existing social security number:

1. Submit a clear copy of your updated and signed Social Security Card and a clear copy of a Government issued Photo Identification* (Drivers License, State ID, Military ID, Passport).

2. Include your contact information (email and phone number) and Student ID number.

3. Submit a copy of your Social Security Card and a Government issued photo identification to:

  • In-Person: Enrollment Services, Haverty Hall 101
  • Mail: NDSCS, Attn: Enrollment Services, 800 6th St, N, Wahpeton, ND 58076
  • Electronic Submission (Encrypted) 
Failure to submit both documents at the same time will result in the request not being processed and the documentation will not be retained.

These steps should only be used by students who do NOT have a social security number on file with NDSCS. If a SSN is already on file, but is incorrect, see steps for submission under Update Social Security Number.

Submit the Taxpayer Identification Request

  • Sign into CampusConnection
  • Select NDSCS eForms tile
  • Select Enrollment Services in the left menu
  • Select 'Taxpayer Identification'
Academic Program Change

Any student who wishes to change their current academic program is required to fill out the Change of Program form.

Family Education Rights and Privacy Act (FERPA)

FERPA is a federal law that governs the rights of students and institutional responsibilities with respect to student records. Under FERPA, some information in a student's record is considered public (directory information) and may be released without the students written permission.

To complete the FERPA release form:

  1. Log in to your CampusConnection account
  2. Click on the eForms tile
  3. On the left-hand side, select Enrollment Services
  4. Select FERPA Release Form
  5. Fill out form and click Submit
Suppress Directory Information

Under the Family Education Rights and Privacy Act, students have the right to request directory information not be made public by notifying NDSCS. Students wishing to restrict directory information must contact the Registrar, Mike Paolini, at 701-671-2204.
