Supplement Information form is required for all international students who are applying for a transfer from a United States Institution to the North Dakota State College of Science. Fill out the first page of the Supplement Information form yourself and return the completed form to Enrollment Services office, 800 North Sixth Street, Wahpeton, ND 58076-0002 or International office or the United States Institution you are now attending or the one you most recently attended. The advisor must fill out the second page. Please hold a conference with your advisor so that he or she can provide accurate information.
