Any student who is going to participate in collegiate athletics at North Dakota State College of Science is responsible for making sure that the Athletic Training Department has the appropriate medical information on file in order for the student athlete to be eligible for competition.
All athletes must submit information to NDSCS via the internet and also present hard copies of the NDSCS physical form, HIPAA form, and also provide proof of insurance to the Sports Medicine Department prior to participation. It is the student-athletes responsibility to keep the Athletic Training Department informed of changes in personal information such as addresses, emergency contracts, phone numbers, and insurance policies.
